Coalition for Compassionate Care of California (CCCC) – provides Californians with numerous resources to educate them on advance care planning and provide the tools to accomplish it. CCCC is a non-profit organization that promotes high-quality, compassionate care for all Californians who are seriously ill or approaching the end of life, and comprised of a statewide collaborative of organizations and individuals representing healthcare providers, assisted living facilities, nursing homes, hospices, consumers, state agencies and others.

The Conversation Project a national project sponsored by the Institutes for Healthcare Improvement and co-founded by award winning columnist Ellen Goodman which provides a “Conversation Starter Kit” to assist individuals in having “the conversation” about end-of-life care with their loved ones, as well as numerous tools to complete advance care planning.

California POLST Information POLST, an acronym for “Physician Orders for Life-Sustaining Treatment,” is an essential document patients and their physicians can complete so that health care professionals honor patient’s treatment wishes once they have had “the conversation” and engaged in advance care planning. The POLST form is recognized under California law and is designed to accompany patients as they receive care throughout the health care delivery system. The POLST form is available on our Forms page.
